The University of Chicago Medicine Comer Children's Hospital offers vaccines for patients who are 6 months and older.

Requests for medical or religious exemption from physical exams and/or immunizations must be submitted in writing. Under Illinois law, the Laboratory Schools administration is responsible for determining the validity of the request.

• Medical exemption is based on a signed statement from your physician specifying the reason for the condition or circumstance.

• Families requesting a religious exemption must complete the Illinois Certificate of Religious Exemption and it must be signed by your healthcare provider.

The University of Chicago Laboratory Schools uses SchoolDoc, a highly secure, HIPAA and FERPA compliant online health records management platform. 

The SchoolDoc platform is secure, encrypted, and password protected. You can find additional information about privacy and security information here. 

• Keep in mind that you can return to app.schooldoc.com at any time to make updates to your student’s health information. Just log in using the email address and password you previously provided. 
• Your student’s health information will carry over from year to year, so once you complete it in SchoolDoc this year, you won’t have to start from scratch next year. 
• SchoolDoc sends out periodic reminder emails when required health information is incomplete. These notifications will come from SchoolDoc.com, so please add this to your safe sender list to avoid accidental delivery to junk and spam folders.

As mandated by the Illinois Child Vision and Hearing Test Act, the Laboratory Schools will conduct hearing screening for Nursery 3–Grade 3 students on a rolling basis throughout the school year. The screening is non-invasive and conducted by a trained technician who has been certified by the Illinois Department of Public Health. If indicated, the screening results will be sent to you with recommendations for follow-up.

The Laboratory Schools will also conduct vision screening for Nursery 3, Nursery 4, Kindergarten, and grade 8 students on a rolling basis throughout the school year. All Nursery 3, Nursery 4, Grade 8 students, and Kindergarten students who have not submitted their IL Vision Form will have their vision screened. Any student who already wears glasses will NOT be screened. The screening is non-invasive and conducted by a trained technician who has been certified by the Illinois Department of Public Health.

Vision screening is not a substitute for a complete eye and vision examination by an eye doctor. Your child is not required to undergo the vision screening if an optometrist or ophthalmologist has completed and signed a report indicating that an eye examination has been administered within the previous 12 months.
