You will receive an invitation from SchoolDoc, Lab's electronic health record management system, to create a health profile for your student. Please complete the following steps to setup and log into SchoolDoc.

1. Click the green “Accept Invite” link within the email to create your SchoolDoc account. This will take you to a screen to confirm your email address and set up your password you will use for this platform.
2. In the Confirm Email box, type your email address in order to confirm.
3. In the Password box, type the password you want to use.
4. Click Continue to finish setting up your student’s health record.
5. After you log in, select your student’s name and click on the health profile link to complete your student’s health record.

Health forms required by Lab | Due by June 8

• IL Health Exam/Immunization Form All non-Lab students are required to provide a copy of their immunization record dated in the past year. 
• Food allergy plan Only for students diagnosed with food allergies. 
  In order to protect the health and safety of students with food allergies, we require that ALL students with allergies requiring emergency medications (Benadryl, EpiPen, inhaler) submit a Food Allergy Action Plan that is completed by a healthcare provider and dated within the past year. Please also review Lab’s Allergy Management Policy for parent/guardian’s responsibility regarding notification, communication, medication, and education.
• Asthma action plan Only for students diagnosed with asthma.
  In order to protect the health and safety of students who have asthma, we require that ALL students with asthma requiring emergency medication, such as an inhaler, submit an Asthma Action Plan that is completed by a healthcare provider and dated within the past year.
• Prescription Medication Authorization Form Only for students who may carry or take prescription medications on school grounds or at school events.
  This form must be completed if your student has (or will have):

  • Any prescription asthma or food allergy medication (ex: albuterol inhaler, EpiPen)

  • Any post-illness prescription medication (e.g., strep throat/pink eye)

  • Any prescription maintenance medicine (e.g., daily ADD/ADHD medication) administered at school, please have your child’s healthcare provider complete this form before the medication can be administered at school. 

    All prescription medication must be in its original prescription bottle, labeled clearly with the student’s name, medication, dosage, and time to administer.

Toileting Expectations

Summer Lab follows the Early Childhood Division’s policies and procedures regarding toileting. All children must be fully toilet-trained to participate in the program. We understand that accidents can happen occasionally and will support children as needed with discretion and care. However, ongoing toileting issues may impact a child’s ability to remain in the program.

To be considered fully toilet trained, a child must be able to:

• Stay acciden-free in regular underwear (no pull-ups) for the full day
• Use the toilet independently with minimal prompting
• Communicate their need to use the bathroom before it becomes urgent
• Manage clothing, wiping, flushing, and handwashing on their own. Teachers will not assist students with wiping. They will guide and prompt them but they will not wipe them.

Arrivals

Families with children at both the Earl Shapiro Hall and Historic campuses typically find it easiest to drop off at Historic Campus first in the morning, then proceed to Earl Shapiro Hall. 

Earl Shapiro Hall  (ESH)
Summer Lab begins classes at 8:30 a.m. Students may arrive at camp no earlier than 8:15 a.m., and will not be allowed to enter the building until that time. 

Students enrolled in a 10:35 a.m. class should arrive no earlier than 10:25 a.m.

All students attending a 10:35 a.m. class should enter through the front entrance of Earl Shapiro Hall on Stony Island Avenue. 

Historic Campus (HC)
Summer Lab opens the Kenwood Mall gates on 59th Street from 8 to 8:15 a.m. For Mid- Morning and Afternoon class arrivals and dismissals, our Summer Lab staff will greet students and open the Kenwood Mall gates. 

Late Arrivals 

Earl Shapiro Hall (ESH)
Students who arrive outside of the arrival window must enter the building through the front entrance on Stony Island Avenue. Parents are expected to walk students to the Summer Lab office in room 202.

Historic Campus (HC)
Students who arrive outside of the arrival windows must enter the building through the Judd entrance at 5835 South Kimbark Avenue. Parents are expected to walk students to the Summer Lab office in Blaine S119. The gates at Kenwood Mall will be locked throughout the day.

Dismissal 

We work hard to support students in their transition from our care to yours, and so we ask that parents/guardians collaborate with us in making this transition as smooth—and often as brief—as possible. Students should be picked up at the time for which they are registered.

Earl Shapiro Hall (ESH)

• The regular pick-up window is from 3:30–3:45 p.m. 
• The midday pick-up window is from 12:35–12:45 p.m.
• Parents and guardians picking up must have placards visible for staff or their unique parent dismissal code from Homeroom.

Carline
When arriving or departing ESH by car, please follow the routes outlined in the appendix. Once your vehicle reaches the designated drop-off and pick-up area—the curb at the south entrance to ESH—you may quickly help your child in and out of the car. Cars must display their carline placard in the dashboard for dismissal. Placards are issued on the first day of camp. 

Walk-up and Indoor Pick-Up
This summer, we are excited to introduce indoor pick-up at Earl Shapiro Hall for all N–2 families. By entering the building for pick-up, you’ll have a valuable opportunity to connect with lead teachers and receive daily updates about your child’s experiences and progress. This approach supports open communication and helps build a stronger partnership between home and school.

• Nursery–Kindergarten (N–K): Pick-up will take place in your child’s classroom, allowing you to see their learning environment and speak directly with teachers.
• Grades 1–2: Pick-up will take place in the ESH lobby, where lead teachers will be available to share insights and answer any questions.
• Late Day Pick-Up: All late day pick-ups will occur in the ESH lobby.

Ramp Procedures 
For both entry and dismissal, please keep to the left side of the two-sided ramp when entering and leaving the building. This procedure ensures smooth and safe movement for all families and students. 

ESH Campus Policies

• Stroller storage and bike racks are available at the main entrance in front of ESH, with additional bike racks located at the back of the building.
• Parents may park in the UChicago parking lot at 60th Street and Stony Island Avenue and walk to ESH.
• Parking is restricted immediately in front of ESH on the west side of Stony Island Avenue, but may be available on the east side of Stony Island Avenue and along Midway Plaisance.
• Visitor parking spots at ESH are reserved for those conducting school business; access to these spots is restricted during pick-up for safety reasons.
• Dogs and other pets are not allowed on campus and should not be brought during arrival and dismissal, regardless of their disposition.

Historic Campus (HC)
Families are responsible for ensuring their child(ren) knows their dismissal plan—that is, self dismissal, using the bus service, carline, or pick-up on foot. 

Carline 
To ensure a safe, efficient dismissal, please remain in your vehicle during the carline on 59th Street. Parents and guardians picking up should display their placards clearly. Our Summer Lab Team will ensure your child is brought to your vehicle. Once your child is safely secured, please promptly exit the carline. 

Walk-up
All students will be dismissed on 59th Street in front of the Kenwood Mall gates, organized by grade. We encourage families walking or cycling to meet their child at their grade-level group. 
 

Summer Lab offers a Late Day program for an additional fee. Please email the Summer Lab office to enroll. 

Please email summerlab@ucls.uchicago.edu with the time and date of your child(ren)’s dismissal. 

Cancellations must have been received in writing by May 15 for a refund minus the $200 non-refundable deposit.

Out of an abundance of caution, we ask that you keep your student home from school if they are experiencing any of the following symptoms and strongly recommend consulting with your healthcare provider to determine if testing for a communicable illness, such as COVID, strep, RSV, influenza, and/or pink eye is appropriate.

Students that are sick may return to school once the following criteria are met:

• Having the energy to participate in a full day of activity
• Fever free for 24 hours without the use of fever reducing medication (if applicable)
• 24-hour cessation of any vomiting and/or diarrhea (if applicable)
• 24-hours have passed since initiation of prescribed antibiotic or antiviral medication (if applicable)
• Improvement of symptoms, but not complete resolution

Absences 

Timely notice from families regarding changes in attendance is essential to the safety and comfort of students participating in Summer Lab. Please email summerlab@ucls.uchicago.edu as soon as you know your child will be absent. 

Please refrain from sending food containing peanuts and tree nuts to Summer Lab. 

Do I get a refund if my child misses camp?
Summer Lab does not prorate or refund for missed days.

Can my child be placed with their friend?
We will do our best to accommodate requests, but we can not guarantee placement.

What if my child does not like the camp?
Please contact the Summer Lab admin team within the first two days of the session. We will work with your family to identify a program that better meets your child's needs.

Who are the instructors and counselors for camp?
Classes are taught by Lab faculty or qualified outside educators. Camp counselors include Lab high schoolers and former campers who have returned to help new campers enjoy Summer Lab as they once did!

Will you post any photos?
Yes. A professional photographer will take photos during camp. Pictures will be posted weekly by campus on the Summer Lab Resource Guide.

How can you help me with my nervous kid?
Summer Lab aims to provide a welcoming environment for all students! Our instructors and counselors are trained to support students as they adjust. Familiarity with many of our returning campers helps us place students where they’ll feel most comfortable.We will also have a dedicated counselor on staff for additional support throughout camp.

What kind of safety do you have on campus or for field trips?
Lab’s security team monitors all campus entrances, and we work in coordination with the University of Chicago Police Department for any extra security needed. During field trips, students are grouped with specific counselors and follow procedures to ensure safety and accountability at all times.

How long do the nursery kids nap? What if my child does not nap?
Nursery students are allotted an hour of their day for rest time. Students who do not rest participate in planned activities with their instructor. 

What happens if my child doesn’t want to attend a field trip?
We ask that children stay home if they do not want to attend a field trip

How do you manage inclement weather days?
We coordinate with Cook County and follow the Chicago Emergency Response System to determine if and when students should remain indoors for the day. Families will be notified of any off-site field trip cancellations, and an alternate indoor curriculum will be implemented if needed.. 

My kid has allergies. How do you keep them safe? 
Our on-site nurse team is available during all program hours. All families fill out their SchoolDoc profile as provided by the Laboratory Schools’ Nurse Division.
