When Your Child Should Stay Home From School
COVID-19 Guidance for When You Test Positive
If you test positive for COVD-19, follow this guidance:
Please notify the nursing team and visit the CDC's website to help determine when you may return to school. Day 0 is the first day that symptoms developed and day 1 would be the first full day after symptoms began (regardless of the date of the positive test). If you never had symptoms you may end your isolation after five days past your positive test.
After five days, you may return to school if the following criteria are met:
- Fever-free for 24 hours without the use of fever reducing medication (if applicable).
- 24-hour cessation of any vomiting and/or diarrhea (if applicable).
- Improvement of symptoms, but not complete resolution.
- Having the energy to participate in a full day of activity.
Upon returning to school, please wear a high quality mask for ten days. Alternatively, you can follow the CDC’s guidelines for testing out of masking with two sequential negative tests 48 hours apart. You can take one on day six and one on day eight. If both are negative, you can remove your mask on day eight.
If you are a close contact to someone who tested positive for COVID
If you consider yourself a close contact to someone who tested positive for COVID, please visit the CDC’s website and click “Isolation & Exposure” to help you determine next steps.
In alignment with updated guidance from the CDC, please do the following:
- If you are having symptoms consistent with COVID, isolate and test.
- If you are not having any symptoms, test on day six after your last contact with the individual.
- If you choose, wear a well-fitted mask for ten days after your last contact with the individual.
Please note that the nurses will not be doing COVID testing at school nor providing tests or masks on a routine basis. Families are encouraged to stock a supply of rapid COVID tests and masks at home.
AEDs on Campus
The University of Chicago Laboratory Schools is equipped with several Automated External Defibrillators (AEDs) on each campus. The AEDs are located

*NOTE: At ESH all the AEDs are located in the short east-west hallway off the main north-south corridor

Grade-level health requirements
Health forms required by the State of Illinois
After you log into your SchoolDoc account, you will be asked to review and accept information previously entered and complete the following health forms required by the State of Illinois. While these forms will be provided within SchoolDoc, samples are linked below.
- IL Child Health Exam Form: due September 5, 2023
A physical examination must be completed within one year prior to entry for:- N3 and Kindergarten (through age 6, a lead screening is also required)
- Grades 6 and 9
- Grade 12 students must submit proof of two MCV vaccinations before starting their year (one received by age 16 or thereafter is also valid)
- Any student newly admitted to Lab.
- IL Dental Exam Form: due March 15, 2024
- Dental exam must be dated on or after November 15, 2022 for:
- Kindergarten
- Grades 2, 6, and 9
- IL Eye Exam Form: due March 15, 2024
- Vision exam must be dated on or after September 5, 2022 for:
- Kindergarten
- Any student enrolling in school in the State of Illinois for the first time
Note: The Child Health Exam form is NOT VALID if the Health History portion is not completed and signed by a parent/guardian.
Caution: theIESA/IHSA athletics physical form will not be accepted in place of a Kindergarten, grade 6, and 9 IL Child Health Exam Form
Health forms required by Lab
The following health forms are required only for those with medically verified conditions. The forms are due September 5, 2023 in SchoolDoc.
- Food allergy plan Only for students diagnosed with food allergies. In order to protect the health and safety of students with food allergies, we require that ALL students with allergies requiring emergency medications (Benadryl, EpiPen, inhaler) submit a Food Allergy Action Plan that is completed by a healthcare provider and dated within the past year. Please also review Lab’s Food Allergy Management Policy for parent/guardian responsibility regarding notification, communication, medication, and education.
- Asthma action plan Only for students diagnosed with asthma. In order to protect the health and safety of students who have asthma, we require that ALL students with asthma requiring emergency medication, such as an inhaler, submit an Asthma Action Plan that is completed by a healthcare provider and dated within the past year.
- Prescription Medication Authorization Form Only for students who may carry or take prescription medications on school grounds or at school events. This form must be completed if your student has (or will have):
- any prescription asthma or food allergy medication (ex: albuterol inhaler, EpiPen)
- any post-illness prescription medication (ex: strep throat/pink eye)
- any prescription maintenance medicine (ex: daily ADD/ADHD medication) administered at school; please have your child’s healthcare provider complete this form before the medication can be administered at school.
- All prescription medication must be in their original prescription bottle, labeled clearly with the name of the student, medication, dosage, and time to administer.
ExemptionsRequests for medical or religious exemption from physical exams and/or immunizations must be submitted in writing. Under Illinois law, the Laboratory Schools administration is responsible for determining the validity of the request.
- Medical exemption is based on a signed statement from your physician specifying the reason for the condition or circumstance.
- Families requesting a religious exemption must complete the Illinois Certificate of Religious Exemption and it must be signed by your healthcare provider.

The University of Chicago Medicine Comer Children's Hospital offers vaccines for patients who are 6 months and older.

Religious and medical exemptions

SchoolDoc Health Records Management
The University of Chicago Laboratory Schools uses SchoolDoc, a highly secure, HIPAA and FERPA compliant online health records management platform.
The SchoolDoc platform is secure, encrypted, and password protected. You can find additional information about privacy and security information here.
- Keep in mind that you can return to app.schooldoc.com at any time to make updates to your student’s health information. Just log in using the email address and password you previously provided.
- Your student’s health information will carry over from year to year, so once you complete it in SchoolDoc this year, you won’t have to start from scratch next year.
- SchoolDoc sends out periodic reminder emails when required health information is incomplete. These notifications will come from SchoolDoc.com, so please add this to your safe sender list to avoid accidental delivery to junk and spam folders.
Screenings
As mandated by the Illinois Child Vision and Hearing Test Act, the Laboratory Schools will conduct hearing screening for Nursery 3–Grade 3 students. The screening is non-invasive and conducted by a trained technician who has been certified by the Illinois Department of Public Health. If indicated, the screening results will be sent to you with recommendations for follow-up.
The Laboratory Schools will also conduct vision screening for Nursery 3, Nursery 4, Kindergarten, and grade 8 students. All Nursery 3, Nursery 4, Grade 8 students, and Kindergarten students who have not submitted their IL Vision Form will have their vision screened. Any student who already wears glasses will NOT be screened. The screening is non-invasive and conducted by a trained technician who has been certified by the Illinois Department of Public Health.
Vision screening is not a substitute for a complete eye and vision examination by an eye doctor. Your child is not required to undergo the vision screening if an optometrist or ophthalmologist has completed and signed a report indicating that an eye examination has been administered within the previous 12 months.

Communicable Disease Reporting
- Notify Lab's nurses when your student has received a positive diagnosis of any communicable disease.
- For N–5 students, Lab will notify relevant families, the teaching team, and administrators if there has been a positive diagnosis of any communicable disease in a N–5 classroom.
