After School: Occasional UseIf your family needs to use any of our Daily Programs (Late Day, Hour After, Quiet Study Hall or Club 58) or the Before School Program on an occasional or emergency basis, please use the form provided below. While availability is limited, in most cases we can accommodate your occasional use of these programs. Online submissions requesting occasional use must be received no later than 10:00 a.m. the day of the use and should not be submitted more than a week in advance. Please be aware that your request is not confirmed until you have received a confirmation email or phone call from the Lab After School office. Of course you can always make these arrangements by calling us at (773) 702-9449. Requests for up to 4 students, 4 programs, 4 dates or any combination thereof can be made on this form. Upon making your selections you only need to click one of the four Submit buttons in the form. Fields marked in bold are required.
