Registration Checklist
Have you completed the registration materials?
Your student will be registered when you:
- Sign the registration form as the parent responsible for payment of tuition.
Make any corrections or additions.
- Provide a social security number for a child who is a citizen of the United States.
- Include a check in the amount of $1000 for the deposit.
- Return to the cashier in the Business Office, Judd 205:

- check for $1000
- deposit form
Exit Form
We will appreciate your kindness in returning the signed exit form if your student
will not be attending the Laboratory Schools for the upcoming academic year.
