On March 1, 2024, the CDC issued new guidance for individuals who test positive for COVID. This guidance was adopted by the IDPH and will be used at the Lab Schools to establish when individuals recovering from COVID may return to school/work. 

Lab community members may return to campus once the following criteria have been met: 

• Fever-free for 24 hours without the use of fever reducing medication 
• Improvement of symptoms, but not complete resolution
• 24-hour cessation of any vomiting and/or diarrhea (if applicable)
• Having the energy to participate in a full day of activity

Upon returning to school and for the next 5 days, the CDC recommends that individuals recovering from COVID follow enhanced hygiene practices, wear a well-fitted mask, and physical distance from others when indoors as much as possible.
 

The University of Chicago Laboratory Schools is equipped with several Automated External Defibrillators (AEDs) on each campus.  The AEDs are located

After you log into your SchoolDoc account, you will be asked to review and accept information previously entered and complete the following health forms required by the State of Illinois. While these forms will be provided within SchoolDoc, samples are linked below.

• IL Child Health Exam Form: due September 5, 2023
  A physical examination must be completed within one year prior to entry for:
  • N3 and Kindergarten (through age 6, a lead screening is also required)
  • Grades 6 and 9
  • Grade 12 students must submit proof of two MCV vaccinations before starting their year (one received by age 16 or thereafter is also valid) 
  • Any student newly admitted to Lab.
     
• IL Dental Exam Form: due March 15, 2024
  • Dental exam must be dated on or after November 15, 2022 for:
  • Kindergarten
  • Grades 2, 6, and 9
• IL Eye Exam Form: due March 15, 2024
  • Vision exam must be dated on or after September 5, 2022 for:
  • Kindergarten
  • Any student enrolling in school in the State of Illinois for the first time

Note: The Child Health Exam form is NOT VALID if the Health History portion is not completed and signed by a parent/guardian.

Caution: theIESA/IHSA athletics physical form will not be accepted in place of a Kindergarten, grade 6, and 9 IL Child Health Exam Form
 

The following health forms are required only for those with medically verified conditions. The forms are due September 5, 2023 in SchoolDoc.

• Food allergy plan Only for students diagnosed with food allergies. In order to protect the health and safety of students with food allergies, we require that ALL students with allergies requiring emergency medications (Benadryl, EpiPen, inhaler) submit a Food Allergy Action Plan that is completed by a healthcare provider and dated within the past year. Please also review Lab’s Food Allergy Management Policy for parent/guardian responsibility regarding notification, communication, medication, and education. 
   
• Asthma action plan Only for students diagnosed with asthma. In order to protect the health and safety of students who have asthma, we require that ALL students with asthma requiring emergency medication, such as an inhaler, submit an Asthma Action Plan that is completed by a healthcare provider and dated within the past year. 
   
• Prescription Medication Authorization Form Only for students who may carry or take prescription medications on school grounds or at school events. This form must be completed if your student has (or will have):
  • any prescription asthma or food allergy medication (ex: albuterol inhaler, EpiPen)
  • any post-illness prescription medication (ex: strep throat/pink eye)
  • any prescription maintenance medicine (ex: daily ADD/ADHD medication) administered at school; please have your child’s healthcare provider complete this form before the medication can be administered at school.
  • All prescription medication must be in their original prescription bottle, labeled clearly with the name of the student, medication, dosage, and time to administer.

ExemptionsRequests for medical or religious exemption from physical exams and/or immunizations must be submitted in writing. Under Illinois law, the Laboratory Schools administration is responsible for determining the validity of the request.

The University of Chicago Laboratory Schools uses SchoolDoc, a highly secure, HIPAA and FERPA compliant online health records management platform. 

The SchoolDoc platform is secure, encrypted, and password protected. You can find additional information about privacy and security information here. 

• Keep in mind that you can return to app.schooldoc.com at any time to make updates to your student’s health information. Just log in using the email address and password you previously provided. 
• Your student’s health information will carry over from year to year, so once you complete it in SchoolDoc this year, you won’t have to start from scratch next year. 
• SchoolDoc sends out periodic reminder emails when required health information is incomplete. These notifications will come from SchoolDoc.com, so please add this to your safe sender list to avoid accidental delivery to junk and spam folders.

As mandated by the Illinois Child Vision and Hearing Test Act, the Laboratory Schools will conduct hearing screening for Nursery 3–Grade 3 students. The screening is non-invasive and conducted by a trained technician who has been certified by the Illinois Department of Public Health. If indicated, the screening results will be sent to you with recommendations for follow-up.

The Laboratory Schools will also conduct vision screening for Nursery 3, Nursery 4, Kindergarten, and grade 8 students. All Nursery 3, Nursery 4, Grade 8 students, and Kindergarten students who have not submitted their IL Vision Form will have their vision screened. Any student who already wears glasses will NOT be screened. The screening is non-invasive and conducted by a trained technician who has been certified by the Illinois Department of Public Health.

Vision screening is not a substitute for a complete eye and vision examination by an eye doctor. Your child is not required to undergo the vision screening if an optometrist or ophthalmologist has completed and signed a report indicating that an eye examination has been administered within the previous 12 months.
