N/K ADMISSION & FINANCIAL AID CHECKLIST




UNFORTUNATELY, DUE TO A DELAY IN LAB'S DATABASE UPDATE, THE ADMISSIONS OFFICE WILL NOT ACCEPT APPLICATIONS UNTIL WEDNESDAY, AUGUST 8, 2012. WE APOLOGIZE FOR THE INCONVENIENCE.  

Nursery 3 applications will be accepted for the 2013-2014 academic year beginning August 8, 2012.

"Part One" of the application process (online application, application fee, birth certificate and parent questionnaire) must be completed (postmarked) by no later than 11:59 p.m. on September 28, 2012
. Please make every effort to submit all "Part One" materials simultaneously, to help with file maintenance. 

Unfortunately, due to space limitations, we ask that families who are interested in Lab’s Nursery 4 and Kindergarten programs join Lab's Interest Pool.
University of Chicago faculty, academic appointees, and candidates/recruits for these positions, as well as siblings of current Laboratory Schools students are permitted to apply to Nursery 4, and are asked to contact the Admissions Office. 

Age requirements for application:

Nursery 3 applicants must be two years old prior to September 1, 2012 (three years old prior to September 1, 2013)
Nursery 4 applicants must be three years old prior to September 1, 2012 (four years old prior to September 1, 2013)


PART ONE - DEADLINE: SEPTEMBER 28, 2012

  Complete and submit Lab’s ONLINE APPLICATION  [Link to be provided August 8, 2012]

      Note: when the application is properly submitted, an email confirmation will be sent to

      the address listed on the application. If no confirmation is received, please re-check 
      the application for missing/incomplete fields! 

 

 Mail the Invoice and $80 non-refundable fee (checks only) to: 1362 E. 59th Street,
             Chicago, IL 60637, Attn: CASHIER

 

  Mail or fax (773-702-1520) the birth certificate (copy) and Parent Questionnaire
             to: 1362 E. 59th Street, Chicago, IL. 60637 attn.: ADMISSIONS

For greater ease of processing, we ask that all applicants submit all "Part One" materials simultaneously. The Admissions Office will mail an acknowledgment letter to each applicant family approximately 7-10 days after receipt of the online application, fee and supporting materials. Once a family has received the acknowledgment letter, then the family should proceed to PART TWO of the application process.

PART TWO - DEADLINE: DECEMBER 31, 2012

  Complete a Parent Interview (Call 773-795-5757 to schedule)

 

  Submit to the child's current school (if applicable):

      a. Records Release Form (download, sign & submit) and
      b. Recommendation Form (download & submit)

Note: schools do not need to finalize recommendation forms until January 15, 2013
       Recommendations are only required if a student attends a daycare or other program
       outside of the home.


 Receive and confirm attendance at your child's assigned Playgroup Date. Please
             confirm attendance two weeks in advance by calling 773-795-5757.
 

     Play dates take place Saturdays and Tuesdays in January and February, 2013.

     The student playgroup is a required part of the admissions process.
     Families are asked to attend their assigned playgroup date. Those who miss the 

     assigned playgroup date will be placed on a playgroup callback list. Unfortunately,

     alternate playgroup dates cannot be guaranteed.


           
        PART THREE - DEADLINE:  FEBRUARY 28, 2013 

  Check with your child’s current school to verify that the recommendation form has been submitted to Lab (January, 2013)

 

 Attend designated Saturday or Tuesday Playgroup (in January or February)

     Admissions decisions will be mailed March, 2013. 

PART FOUR - DEADLINE: FEBRUARY 1, 2013 (optional)

  Apply for Financial Aid

Note: the deadline for completion of the financial aid process is February 1, 2013.  For full

consideration, please adhere to this date.

 

An application to Lab does not guarantee admission. The Admissions Office recommends that all applicants explore a variety of educational options.  All applicants, including members of the University of Chicago community, remain subject to classroom space constraints and stated application deadlines.  If you are applying to any grade outside of the application period, please contact the Admissions Office to discuss the status of late applications. Final admission decisions will not be made until all documents have been submitted, and the Admissions Committee has reviewed the completed file.
