Admission Documents

Recommendation forms, provided by the Lake Michigan Association of Independent Schools (LMAIS) and common to the admission processes of said schools, are a required component of all applications to the University of Chicago Laboratory Schools. When forwarding these forms to the appropriate school personnel, applicants need to remind each school representative completing the form(s) that (s)he should keep the original, then send photocopies to only those schools requested by the applicant and listed on the last page of the LMAIS Common Recommendation form. Please be aware that each school may have different conditions under which they will accept recommendation forms, and that the information provided here applies only to the UC Laboratory Schools' admission process.

Common Recommendation Forms

PLEASE REVIEW THE FOLLOWING BEFORE DOWNLOADING THE APPROPRIATE FORM(S).

As with the admission processes, each grade level has slightly different requirements for the submission of these forms. Common to all forms are the following:

1. Recommendation forms are confidential between the applicant's current school and the Laboratory Schools, and must be submitted by the school or teacher, not the parent. The Lab Schools' Office of Admission will not accept recommendation forms faxed, mailed, or "walked in" by anyone other than a school representative, unless the forms are in a sealed envelope and signed across the seal by a school representative.
2. Forms must be completed by the applicant's current teacher (or other school personnel, as indicated). Recommendations completed by a previous year's teacher or principal do not meet this requirement.
3. Forms must be completed and submitted after December 14, 2007. The Office of Admission will return all recommendation forms submitted before this date.
4. Other recommendation forms completed by the current school, letters from current teachers or other school personnel, or personal recommendations do not fulfill the "Recommendation Forms Submitted" requirement. However, the Office of Admission will accept the above as supplemental recommendations.

FOR APPLICANTS TO:

Nursery School (1 form) — Please have the head teacher complete and return this form to the Lab Schools by January 18, 2008.
Applicable only to those children currently enrolled in a school or formal daycare program.
